**This Position is Eligible for a Sign-On Bonus of $2,000** $1,000 payable on next scheduled paycheck following 30 days of employment and $1,000 payable at the completion of the conditional employment period which is typically six months in length.

Work involves clinical screenings, assessments, and determinations for hospitalization and/or ongoing service eligibility based on consumer needs both in-office and within the community.
Coordinates, links, and authorizes appropriate services and monitors consumer progress with meeting stabilization goals.
Works under limited supervision with considerable latitude for the use of initiative and independent judgment.
This position provides a majority of services in the community, such as homes, jail settings, shelters, hospitals, and other environments in which the client lives and thrives.
Due to this, this position is required to travel frequently within the service area and may interact frequently with clients, families, law enforcement/justice system officials, hospital staff, and other community stakeholders involved in the recovery of the individual they are serving.
ESSENTIAL DUTIES AND RESPONSIBILITIES Serves as the subject matter expert (SME) in children’s needs and services for the crisis response teams at Central Counties Services.
Attends regular training and disseminates materials regarding children in crisis.
Creates and maintains relationships with referral sources such as schools, pediatricians, and DFPS.
Serves as a consultant to crisis team members regarding children in crisis.
May assess walk-in individuals currently not in ongoing Central Counties’ services who present in crisis.
Conducts crisis assessments for individuals who present in crisis in Central Counties’ hospitals, jails, and detention facilities for treatment recommendations in the least restrictive environment.
Make appropriate determination for client placement depending on level of current risk based on information gathered from crisis assessment and Columbia Suicide Severity Rating Scale.
If client is determined to meet criteria for an outpatient level of care, then a safety plan will be created or reviewed on-site with the client.
Documentation in agency’s Electronic Health Record that a safety plan was completed is required.
A copy of the safety plan is to be provided to the client.
Serve as a linkage into outpatient care, to include facilitating intake interview, coordination with LPHA for diagnostic recommendation, development of person-centered recovery plan, and referral to indicated services.
Conducts crisis assessments for State Mental Health Facility (SMHF) placement, upon request.
Provide hospital discharge follow up within seven days, crisis follow up and transition services to individuals.
Collaborates with all community stakeholders regarding mental health crisis concerns.
Identifies problem areas, service gaps, and needs that are barriers to accessing or participating in services by assessing individuals’ support systems and available community resources, schools, transportation, etc.
, and making necessary referrals and plans to meet identified needs and outcomes.
Meet crisis response timeliness requirements for assessment requests as identified by the State of Texas: Emergent within one hour, Urgent within eight hours, and Routine within twenty-four hours.
Meets all clinical needs and minimum service requirements according to Center staff training requirements, Resilience & Disease Management Design and Texas Administrative Codes.
Documents case records, assessments, person-centered recovery plans and progress notes according to Center requirements and Texas Administrative Codes within two business days.
Participates in quarterly training and time studies for Random Moment Time Study/Medicaid Administrative Claiming (MAC).
Work behavior must be compatible with Center value statement, Policies, and Procedures Manual.
Performs related work as assigned by Supervisor.
Must stay compliant with all required training.
Demonstrate high ethical standards, trustworthiness, and personal integrity.
Regular attendance, dependability, and promptness are required for the 8-5 workday or agreed upon hours 100% of the time, to ensure consistency and completeness of program’s processes.
Professional Development - Successfully complete training required to maintain skill competency and cross training to ensure consistency across program procedures.
Complete continuing education as required by professional standards and the Center.
Frequent daily driving within the catchment area is required.
GENERAL QUALIFICATIONS Experience/Education/Licensing/Certification Experience in a field relating to the agency program.
Graduation from an accredited four-year college or university with major course work in a field relevant to the assignment.
Experience and education may not be substituted for one another.
