Works with Service Coordinators, provider and other service agencies to identify individuals with IDD who are at risk of requiring crisis services.
Collaborates with team members to identify prevention strategies, training and supports and to promote coping skills.
Provides education and consultation to the Mobile Crisis Outreach Team and law enforcement regarding individuals with IDD.
Works closely with IDD Crisis Respite services to develop a therapeutic plan for identified individuals.
Coordinates paperwork for admission and discharge for all crisis respite services.
Responsible for completing reporting requirements set forth by DADS Performance Contract.
*ESSENTIAL DUTIES AND RESPONSIBILITIES* *Assessment: *Responds to referrals for Crisis Intervention Services within 48 hours.
Completes initial screening/assessment to determine level of support needed and appropriate placement for Crisis Intervention Services or 90-day wraparound services.
Reassesses clients assigned to Crisis Intervention Services or 90-day wrap-around services to determine progress or need for continued services.
Collaborates with various providers to identify individuals with IDD who are at risk of requiring crisis services.
Consults with Center Board Certified Behavior Analysts and Licensed Professional Counselor on cases identified as outside the scope of abilities.
*General Education, Training and Consultation: *Provides information about IDD programs and services to individuals with IDD and their families.
Provides education and consultation to Mobile Crisis Outreach Teams, law enforcement, emergency departments and other agencies.
Provides education regarding clients with IDD and manner in which to engage their unique needs.
Collaborates with Service Coordinator, Behavior Supports, Specialized Therapies and other provider services to ensure coordination and monitoring of services.
*Client Specific Education, Training and Consultation: *Provides, or arranges for the provision of skills training and client specific education regarding diagnoses and conditions.
Facilitates education sessions with client, families and caregivers.
Identifies prevention strategies to avoid potential crisis events and to promote coping skills.
*Treatment Planning: *Interviews clients, families, providers, and others involved to obtain needed information.
Consults with Service Coordinator, Behavior Supports, Specialized Therapies and other provider services to assist in the development of a treatment plan.
Identifies training and support needs that provide the greatest chance of success in the community.
Responsible for completion of the therapeutic support plan for clients utilizing crisis respite services.
Responsible for providing discharge planning for clients leaving crisis respite services.
Collaborates with the Crisis Service Coordinator for clients discharging from 90-day wrap-around services.
*Crisis Respite: *Assists in overseeing both hourly and daily Crisis Respite program.
Crisis Respite duties include: * Coordinates admission and discharge for crisis respite services, as well as coordinates and schedules the delivery of crisis respite with Behavior Support Supervisor.
* Provides support to staff providing crisis respite services.
May facilitate individual or group training while in respite.
Coordinates provision of crisis respite with contracted daily respite providers.
Provides training, in conjunction with BCBA and Specialized Therapies, to daily respite providers.
* Responsible for the admission and discharge planning duties of all clients entering hourly or daily crisis respite services.
* Maintains master appointment schedule for all clients who are receiving crisis respite services.
This includes coordination of any scheduled appointments that may be occurring while an individual is in daily crisis respite services.
* Prepares and maintains client’s folders to ensure that all supporting documentation is received prior to respite services beginning.
This can include physician’s order, nursing assessment and special needs packet, delegation of medications, relevant person-directed plan, crisis respite plan or implementation plan.
This list is not inclusive of all documentation that may be required.
* Liaisons between Crisis Intervention Team and other members of the client’s support team as to the status of services.
* Liaisons with families, caregivers, and other providers as it relates to admission, service delivery and discharge planning.
* Can be responsible for providing respite care in either hourly or daily respite as needed.
When providing direct care, will carry out the same responsibilities, such as: providing therapeutic supports and strategies identified on the crisis respite plan, ensuring daily living needs are met (hygiene, meal preparation, etc).
* Works with crisis support team to ensure the delivery of therapeutic activities occurs as determined by the individual's Crisis Respite Plan.
* May provides transportation, in a Center vehicle, to appointments or other activities.
* If applicable, follows medication delegation and documents the consumer taking the medication(s) as indicated.
* Follows an individual's behavior management plan.
* Completes incident reports no later than the end of the next business day.
* May assists the Program Manager with special projects.
* Maintains crisis respite unit equipment and requests repairs as needed.
*Documentation-*completes assessments, treatment/therapeutic plans, progress notes in a timely manner.
Provides written feedback to referral source as to the status of referral for Crisis Intervention Services.
* Responsible for submitting service tickets/progress notes within three calendar days to data entry.
*Administrative Tasks: *Completes required quarterly reporting due to HHSC accurately and on time.
Maintains a network of providers for daily crisis respite services.
Assist with oversight of contracts for daily respite providers.
Ensures paperwork for daily crisis respite is received in time to meet encounter deadlines.
Works closely with IDD Data Manager to ensure credit.
Completes Purchase Orders to ensure reimbursement is provided for contracted Crisis Respite providers.
Assist IDD Director with budget processes related to the delivery of crisis respite services.
Provides evaluation and recommendations to IDD Director as it relates to Crisis Intervention Services and Crisis Respite.
Develops a crisis respite service plan for therapeutic support.
*Travel: *Travels the 5 counties served.
*After-hours work: *May be required to be available outside normal working hours for consultation with MCOT or daily respite providers.
*Other Work Requirements:* * Must stay compliant with all required training.
* Demonstrate high ethical standards, trustworthiness, and personal integrity.
Regular attendance, dependability, and promptness are required for the 8-5 workday or agreed upon hours 100% of the time, to ensure consistency and completeness of program’s processes.
* Professional Development - Successfully complete training required to maintain skill competency and cross training to ensure consistency across program procedures.
* Frequent daily driving within the catchment area is required.
*GENERAL QUALIFICATIONS* *Experience/Education/Licensing/Certification* * Bachelors degree in psychology, sociology, social work, criminal justice, or a related human services field.
* Has experience working with individuals with IDD in crisis.
*Knowledge/Skills/Abilities* * Knowledge of the principal practices of behavior analysis or related discipline.
* Ability to develop and determine effectiveness of behavior support plans for individuals with developmental disabilities * Ability to complete and interpret data reports.
* Ability to assess crisis situations quickly and determine needed supports.
