69 hrs per pay period 10 Observed Company Paid Holidays 8 Hours Annual Volunteer Time Off Retirement Plan w/ Employer Match Employee Assistance Program Flexible Spending Account GENERAL DESCRIPTION Under the direction of the HCS Program Manager and within federal, state and local guidelines performs entry-level to routine habilitative and rehabilitative care work with individuals who have developmental disabilities.
ESSENTIAL DUTIES AND RESPONSIBILITIES The Direct Support Staff provides direct care and services to persons with developmental disabilities.
The Direct Support Staff is available to the individuals served during their assigned shift to provide ongoing availability to the individuals served.
Duties consist of quality interaction and support of individuals in all activities of daily living, personal care, routine housekeeping, cooking, shopping, and community integration.
Responsibilities also include emergency behavior or health related interventions when needed.
Serves as a positive role model for individuals served and represents the Center in the community when on duty.
Direct Care Staff may be assigned to any home within their assigned shift based on organizational need.
Direct Care Staff designated as a "floater" position may be assigned to any home within their assigned shift based on organizational need.
The assigned shift may vary and may include any week/weekend day, evening, or night shift.
Works under moderate supervision, with limited latitude for the use of initiative and independent judgment.
This position will include transportation to individuals served between residence and Fun Night on Thursday evenings.
May also include providing community support services to individuals who reside in the community.
Interaction with individuals served: Develops rapport with each individual served in a manner that demonstrates respect and an approach that centers on each persons goals and preference.
Consistently asks individuals their preferences and consistently acts upon those preferences.
Communicates and acts in a manner that supports the rights of individuals.
Utilizes communication techniques, including verbal Prevention and Management of Aggressive behavior (PMAB) and behavior management methods, to prevent need for more intrusive measures of behavior intervention.
Actively protects individuals from abuse, neglect, and violations of their rights.
Interactions with individuals served demonstrate and promote dignity and respect.
Ensures safety of individuals served by providing adequate supervision to ensure health and safety needs are met and/or reported for each individual served.
Provides supervision according to individual Implementation Plan Reports health or safety concerns to nursing and supervisory personnel immediately.
Completes incident and medication errors reports as needed Training and program implementations: Is knowledgeable and follows Individual Implementation Plans and Behavior Management Plans or Behavior Guidelines.
Completes all documentation required for program implementation including training objectives, behavior data cards and observation notes.
Must stay compliant with all required training.
Incorporates training identified in the individual plans into daily activities as required.
Financial Assistance: Assists individuals served with money, record keeping, and purchasing as required per Center procedures.
Assists individuals served with purchases.
Retains and submits all receipts per center guidelines Ensures appropriate documentation completed for all transactions.
Assists individuals served with monies as stated in individual served Implementation Plan (IP).
Emergency intervention: Provides emergency intervention when necessary according to DADS and Center rules and procedures.
Provides verbal and physical PMAB intervention correctly when necessary.
Provides CPR in an emergency until other medical aid arrives.
Provides emergency and non-emergency first aid until professional medical aid arrives or as directed by licensed medical professional.
Maintains all emergency intervention training according to Center policies and procedures.
Manages difficult and challenging behaviors according to individual behavior management plans.
Assists individuals with administration of medications: Provides training and administration of medications according to Center policy and instructions from nursing services.
Assists individuals in administering their medications in a manner that maintains sanitary conditions.
Assists and trains individuals in administering their medications according to their physician's orders and IP.
Accurately documents all medication administration 100% of the time.
Ensures that medications are properly secured 100% of the time.
Follows Center procedures for reporting medications needed for reorder, or if medications or nursing supplies are unavailable.
Completes medication incident/error reports if medications are not taken or documented according to physician's orders.
Administers medications only in accordance with tasks delegated by Registered Nurse.
Home maintenance and cleaning: Assists individuals in housekeeping duties to the extent needed and completes remaining cleaning tasks as assigned.
Encourages individuals to clean, uses demonstration and other techniques to teach individuals to maintain their home in a healthful and safe manner.
Cleans and assures that individuals served clean in a manner that uses equipment and chemicals safely.
Completes household cleaning tasks unable to be completed by individual served, and as assigned by supervisor.
Maintains a safe, sanitary and clean environment at all times.
Including but not limited to floors, furnishings, bathrooms, kitchen, common areas, living areas and individuals served bedrooms.
Reports maintenance and supply needs to designated personnel.
Documentation: Completes documentation that reflects service provision including observation notes, incident reports, and behavior data cards, training data cards, medication administration records and log book.
Writes legibly and in a manner that can be understood by others.
Ensures documentation is completed within time lines 100% of the time.
Ensures documentation is accurate 100% of the time per HCS billing guidelines Provides Transportation for individuals, as required, maintaining Center guidelines and State laws.
Safely transports individuals both in assigned areas using Center vehicles without accident, citation or other incident.
Follows all Center guidelines while driving, including restriction of food, drink, smoking, and cell phone use.
Adheres to all state driving laws.
Ensures vehicles are maintained with appropriate amounts of fuel and fluids.
Completes all required vehicle checklists as scheduled.
Maintains fuel and fluid levels of vehicles and reports all maintenance issues immediately to the appropriate supervisor.
Maintains personal driving record within agency standards and requirements.
Provides ongoing evidence of current auto liability insurance.
Reports all vehicle incidents immediately to supervisor or on-call personnel.
Reports all vehicle maintenance issues to immediate supervisor.
Keeps vehicles free from trash, debris, spills and stains.
Assists individuals served with meal planning and food preparation in accordance with individual choices and needs.
Respects individuals choices related to food.
Encourages individual's independence in food preparation by assisting only to the extent required by individual served need.
Supervises meal preparation to ensure hand washing, dish washing, and food handling techniques are maintained to ensure proper food sanitation.
Follows physician and/or Team recommended diet in assisting individuals served plan menus.
Ensures nutritious balanced meals are provided.
Purchases, prepare and serve meals according to budget, individual dietary needs, and dietary instructions, playing close attention to specialized diets and textures.
Monitors meals to prevent choking or interpersonal conflicts during meals.
Assists individuals served in shopping for supplies, groceries and personal items following guidelines of individual served choice and all purchasing regulations.
Makes only purchases from approved lists or assignments from supervisor.
Purchases groceries according to diets.
Ensures designated guidelines are followed when assisting with personal shopping, including collecting and submitting required receipts per Center guidelines.
Reliability Completes tasks within assigned time lines.
Is consistently available to support individuals served so that programs and services occur without interruption.
Communication Ensures that internal communications follow departmental procedures with respect to protocol and lines of authority.
Advises supervisor promptly of incidents or situations which would have an unusual effect, either positive or negative, on the agency's mission, productivity, or direct services.
Customer service Promotes principles of customer service through respectful and professional interactions with individuals served, family members, the community and staff in a manner that supports teamwork and all other aspects of the mission of the Center.
Adheres to all Center Standards of Conduct.
Adherence to state and federal laws, regulations, and agency policy.
No deviations from laws, regulations, policies, or procedures pertaining to the operation of the Center.
Reports any deviations as needed to the supervisor.
Reports allegations or suspicions of abuse, neglect, or exploitation per Center policy.
Attends all required training on time and demonstrates competency in all required skills, per Center policy.
Work Safety and Health Adheres to all emergency and preventative safety procedures in the performance of all work duties.
Identifies and immediately reports safety hazards to designated personnel.
Completes incident reports for all on-the-job injuries and reports to designated personnel on the shift the injury occurred.
Travel Frequent daily driving within the catchment area is required.
GENERAL QUALIFICATIONS Experience/Education/Licensing/Certification High school diploma or equivalent.
Six (6) months experience working with individuals with disabilities preferred.
Registration, Certification, Licensure or Other Qualifications All job specific training must be completed within 30 days of hire and upon renewal if required for position.
Must maintain a valid driver's license, auto liability insurance and a driving record acceptable to the Centers insurance requirements.
Certification in First Aid, CPR and PMAB.

Knowledge of rehabilitative programs for individuals with developmental disabilities preferred.
Skill in the operation of computers and applicable computer software.
Ability to provide resident care; to prepare and maintain records both electronically and on paper, to administer first aid or cardiopulmonary resuscitation (CPR), to conduct prevention and management of aggressive behavior (PMAB), and to communicate effectively.
Relationship Skills Ability to communicate effectively, both verbally and in writing.
