Hospice Case Manager General Purpose: To assure quality patient care and services are provided to in-home clients.
Accountable for making decisions based on his/her educational preparation and experience in nursing.
Responsibilities include:
Assessing and providing hands on care
Admissions after hours
After hours discharges including death pronouncement
Communicating with the interdisciplinary team
Documenting necessary information in the electronic healthcare record
Collaborating with the hospice physician
Coordinating necessary ancillary services such as DME, lab, x ray, transportation
Providing services collaboratively with other healthcare providers such as in nursing facilities, hospitals and home health
Oversight and direction of unlicensed assistive personnel
Education and support for family, caregivers, and facility staff
Hospice Case Manager Minimum Education & Experience Requirements:
Current RN License – active and in good standing
Minimum of 1-year nursing experience preferred
Hospice experience is a plus
Current CPR certification
Strong organizational and communication skills
Proof of current auto insurance
