JOB SUMMARY
The Licensed Vocational Nurse (LVN) is a licensed nurse who uses the BSW nursing professional practice model to deliver patient care as part of the health team.
The LVN In Basket performs appropriate tasks, for top of license/role, associated with the in basket and patient electronic medical records.
ESSENTIAL FUNCTIONS OF THE ROLE
Addresses in-scope, patient messaging using standard workflows.
Serves as initial reviewer of all in basket messages for assigned care team.
Identifies messages appropriate for top of license/role.
Inserts patient education as instructed by providers.
Marks status of in basket messages appropriately so other care team members can identify areas of ownership.
Matures and routes messages not in scope using a variety of resources including chart review and contacting patients.
Serves as initial owner of care cap closure for assigned patients.
Performs basic scheduling tasks to include same day appointments, next day appointments and lab re-checks.
Coordinates referrals and prior authorizations (per clinic workflow).
Completes FMLA, home health and other paperwork.
Executes standing delegated orders.
Addresses pend/plan orders and associated diagnosis codes.
Documents clearly and efficiently in the electronic medical record.
Enters external test results into electronic medical record.
Makes patient calls and my chart notifications; accurately gathers and documents pertinent clinical information from patient to provider; accurately relays clinical information/education from provider to patient.
May perform LVN in-clinic duties as outlined in the LVN job description.

Ability to successfully manage in basket activity per scope and licensure.
Knows and adheres to in basket productivity measures and turn-around time targets.
Knowledge and expertise of nursing and patient care standards and procedures.
Knowledge of laws, rules, regulations; standards and guidelines certifying and accrediting bodies, hospital and department/unit standards, protocols, policies and procedures governing the provision of nurse care applicable to the area of assignment.
Knowledge of medical terminology; methods and practices of health promotion, risk reduction, illness and disease prevention and management; medication and drugs, common dosages their physical and physiological effects, and possible adverse reactions.
Knowledge of medical and professional nursing ethics and patient privacy rights.
Must be able to communicate thoughts clearly, both verbally and in writing, with professional demeanor.
Must have critical thinking and problem-solving skills.
Ability to watch changes in the medical condition of patients and effectively communicate these changes to other nursing staff members and physicians/providers.
Ability to give age-specific, quality, patient-centered care to all patients through the nursing process and standards of nursing practice with care and respect of for the diversity of human experience.
Ability to promptly assess requests by using electronic and paper resource materials and correctly respond to patient inquiries.
Excellent data entry, numeric, typing and computer navigational skills with attention to details.
Relaxed working in a fast-paced environment.
